2. Electronic or physical signature of the owner or authorized person; 
3. Detailed description and location of the infringed work;
4. Good faith statement indicating unauthorized use;
5. Confirmation, under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

To file a counter notification for Intellectual Property Rights infringement, you must include the following information:-

OUR POSITION

If we receive a valid claim of Intellectual Property Rights infringement, we will remove or restrict access to the material and notify the subscriber. If the subscriber submits a valid counter-notification, we will inform the original claiming party and provide them with a copy of the counter-notification. Suppose the original claiming party doesn't respond within 9 to 14 business days. In that case, we will replace the removed material or cease disability access unless there's a court order to restrain the counterparty from infringement. The laws of the USA and the United Republic of Tanzania govern the policy.
